Reporting and Outcomes of Brachytherapy for Gynecologic Re-irradiation: A Systematic Review
This educational activity explores the role of gynecologic re-irradiation using brachytherapy (GRIB) as a salvage treatment option for patients with loco-regionally recurrent gynecologic cancers who have previously received radiation therapy. The article of focus is a review paper analyzing 50 publications and nearly 2,000 patients, indicating that modern image-guided brachytherapy techniques can achieve meaningful local control and survival outcomes in carefully selected patients, although significant risks of severe late toxicity remain. The review also highlights major limitations in the current evidence base, including heterogeneous treatment approaches, small retrospective studies, and inconsistent reporting of key clinical parameters. Additionally, dose accumulation for these patients is challenging due to evolving pelvic anatomy and uncertainties in radiobiological models for re-irradiation. The authors highlight that to strengthen the evidence necessary to guide future clinical decision-making around GRIB, more standardized reporting, robust dose summation models, and multi-institutional clinical and radiobiological research are needed.
