Role of Radiation in Oligometastatic and Polymetastatic Disease Webinar Series

Radiation therapy continues to make significant strides in the management of early-stage and locally advanced solid tumor malignancies. Ablative radiation therapy or SBRT is routinely utilized for oligometastatic solid tumors, and there are ongoing trials looking at expanding indications to polymetastatic disease. There are multiple advances in single-fraction SBRT, repeated courses of SBRT, surrogate endpoints for metastasis-directed therapy, identification of biomarkers of response, and pushing the boundaries of the number of metastases that can be safely treated. This webinar series will discuss advancements in metastasis-directed therapy, current indications and challenging cases.  

This series is divided into two one-hour sessions, one focusing on SBRT for oligometastasis and the other on polymetastatic disease.

Role of Radiation Therapy in Oligometastatic Malignancies

This presentation will review the evolving role of radiation therapy in the management of oligometastatic malignancies, with an emphasis on prostate, renal, and urothelial cancers. The talk will highlight the biological and clinical rationale for metastasis-directed therapy, summarize key prospective and retrospective evidence supporting the use of stereotactic body radiation therapy and other focused radiation approaches, and discuss how patient selection, disease burden, imaging advances, systemic therapy integration, and treatment timing influence outcomes. Special attention will be given to the distinction between oligorecurrent, oligoprogressive, and de novo oligometastatic disease, as well as the potential
goals of radiation therapy, including delaying systemic therapy, improving progression-free survival, consolidating response, and palliation. The presentation will also address practical considerations for multidisciplinary decision-making, toxicity mitigation, and future directions, including biomarker-driven selection and ongoing clinical trials aimed at refining
the use of radiation therapy in this increasingly important clinical setting.
 


Evolving Role of Radiation in Polymetastatic Disease

This webinar will provide a practical medical physics perspective on implementing SBRT for polymetastatic disease. Key topics will include CT simulation and imaging strategies, motion management, multi-isocenter treatment planning, composite plan evaluation, and treatment delivery considerations. Drawing on practical examples and lessons learned
from the ARREST and ARREST-2 trials, the presentation will highlight how structured multidisciplinary communication, quality assurance checkpoints, and adaptive decision-making can support safe, efficient, and high-quality treatment delivery.
The presentation will also review the clinical rationale for extending SABR to polymetastatic disease, examine emerging evidence from the ARREST studies, and discuss practical
considerations for safely implementing these complex treatments. Topics will include patient selection based on technical feasibility rather than lesion count alone, multidisciplinary workflow, simulation and treatment delivery strategies, organ-at-risk–driven decision making, and planning approaches that enable treatment of numerous metastases while maintaining safety. Attendees will gain practical insights into how comprehensive SABR can be incorporated into clinical practice and how lessons from the ARREST program are informing the ongoing international ARREST-2 trial.
