2025 ASTRO Coding and Coverage Seminar - 3D, IMRT and Brachytherapy

Topics Included:

1. 3D and IMRT
This content covers the differences as it related to clinical work and coding for forward planning, inverse planning, compensators, and the “Choosing Wisely” criteria. Currently, there is a knowledge gap in coding regarding the differences between 3D and IMRT.

2. Brachytherapy
This content reviews the radiation therapy modality of Brachytherapy. Codes specific to this modality are addressed.  ASTRO’s Brachytherapy Model Policy is reviewed in detail.

This activity was originally presented and recorded at the 2025 ASTRO Coding and Coverage Seminar.

This activity is available from January 28, 2026, through 11:59 p.m. Eastern time on March 14, 2026.

Target Audience

This activity is designed to meet the interests of radiation oncologists, physicists, radiation therapists, radiation dosimetrists, practice administrators, certified coders and professional coders.

Learning Objectives

Upon completion of this activity, participants should be able to:

  • Summarize the differences in coding for forward planning and inverse planning for IMRT courses of treatment.
  • Identify codes that are included in the work for CPT 77301 and are not separately reimbursable.
  • Explain the intricacies of a 3D plan and how it differs from an isodose plan.
  • Properly use and code for image guidance associated with these plans.
  • Explain the complexities associated with Brachytherapy.
  • Explain the role of the Nuclear Regulatory Commission and other agencies involved with Brachytherapy treatments due to the use of radioactive isotopes.
  • Identify differences in LDR and HDR Brachytherapy and how to correctly code the treatments.
  • Navigate a combined brachytherapy/external beam RT treatment plan with correct coding.
